I am pretty sure the implementation with javascript and the checkbox is causing the issue.

There is no actual button. Can you try putting an actual button there just for the test? If it works I will guide you how to do that javascript implementation again, but you most likely won't need it as I can see the site is done in a professional manner. Good job.

I don't want to put the code myself as the editor might do cleanup and wipe your old code.
